Delaware County Health Department Community Health Survey

  • The Delaware County Health Department is conducting a Community Health Survey, which is available for completion until June 30th for all adults who live, work, go to school, or receive services in Delaware County. The survey takes approximately 15 minutes to complete. Survey responses will help the Delaware County Health Department determine community health priorities for Delaware County health initiatives. Your response helps make our communities healthier.
